Many Student Affairs divisions establish operational plans to continually improve their student, and in some cases community, services. They can be planned and assessed on an annual basis or over a period of several years. Attached is a great example of a Division of Student Affairs Operational Plan.

The example provided in this article is from a public, 4-year state university, the Departmental Mission and Vision, as well as Core Function are reported by the "Parent" department. For example, Student Involvement could be a "child" department of the Dean of Students department. Then, a "child" department would report on Primary Functions, Intended Outcomes, and Action Steps. The Planning templates are in the attached document, Division Operational Plan. A comprehensive report can appear as the attached example, Dean of Students Operational Plan.
